适应症
本品用于。1. 预防和治疗肿瘤放疗或化疗后引起的白细胞减少症。 2. 治疗骨髓造血机能障碍及骨髓增生异常综合症。 3. 预防白细胞减少可能潜在的感染并发症。 4. 使感染引起的中性粒细胞减少的恢复加快。
用法用量
1. 肿瘤放、化疗后、放、化疗停止24～48小时后方可使用本品,用1ml注射用水溶解本品(切勿剧烈振荡),在腹部、大腿外侧或上臂三角肌处进行皮下注射(注射后局部皮肤应隆起约1cm2,以便药物缓慢吸收),3～10μg/kg/日,持续5～7天,根据白细胞回升速度和水平,确定维持量。本品停药后至少间隔48小时方可进行下一疗程的放、化疗。2. 骨髓移植、5～10μg/kg,静脉滴注4～6小时每日1次,持续应用至连续3天中性粒细胞绝对数≥10ｏ0/ml。3. 骨髓增生异常综合征/再生障碍性贫血、3μg/kg/日,皮下注射,需2～4天才观察到白细胞增高的最初效应,以后调节剂量使白细胞计数维持在所期望水平,通常＜10000/μl。
不良反应
本品的安全性与剂量和给药途径有关。大部分不良反应多属轻到中度,严重的反应罕见。最常见的不良反应为发热，寒战，恶心，呼吸困难，腹泻,一般的常规对症处理便可使之缓解;其次有皮疹，胸痛，骨痛和腹泻等。据国外报道,低血压和低氧综合症在首次给药时可能出现,但以后给药则无此现象。不良反应发生多于静脉推注和快速滴注以及剂量大于32μg/kg/日有关。
禁忌
1. 对rhGM-CSF或该制剂中任何其他成份有过敏史的病人。 2. 自身免疫性血小板减少性紫癜的病人。
注意事项
1. 本品应在专科医生指导下使用。病人对rhGM-CSF的治疗反应和耐受性个体差异较大、为此应在治疗前及开始治疗后定期观察外周血白细胞或中性粒细胞、血小板数据的变化。血象恢复正常后立即停药或采用维持剂量。 2. 本品属蛋白质类药物、用前应检查是否发生浑浊、如有异常、不得使用。 3. 本品不应与抗肿瘤放、化疗药同时使用、如要进行下一疗程的抗肿瘤放、化疗、应停药至少48小时后、方可继续治疗。 4. 孕妇、高血压患者及有癫痫病史者慎用。 5. 使用前仔细检查、如发现瓶子有破损、溶解不完全者均不得使用、溶解后的药剂应一次用完。
