适应症
本品适用于敏感菌所致的呼吸道感染、胃肠道感染、尿路感染、软组织感染、心内膜炎、脑膜炎、败血症等。
用法用量
1.成人:肌内注射一日2～4g,分4次给药;静脉滴注或注射剂量为一日4～8g,分2～4次给药。重症感染患者一日剂量可以增加至12g,一日最高剂量为14g。
2.儿童:肌内注射每日按体重.0～100mg/kg,分4次给药;静脉滴注或注射每日按体重100～200mg/kg,分2～4次给药。一日最高剂量为按体重300 mg/kg。
3.足月新生儿:按体重一次12.5～25mg/kg,出生第1、2日每12小时1次,第三日～2周每8小时1次,以后每6小时1次。
4.早产儿:出生第一周、1～4周和4周以上按体重每次12.5～50mg/kg,分别为每12小时、8小时和6小时1次,静脉滴注给药。
5. 肾功能不全者:内生肌酐清除率为10～50ml/分钟或小于10ml/分钟时,给药间期应分别延长至6～12小时和12～24小时。
6.氨苄西林钠溶液浓度愈高,稳定性愈差。在5℃时1%氨苄西林钠溶液能保持其生物效价7天,但5%的溶液则为24小时。浓度为30mg/ml的氨苄西林钠静脉滴注液在室温放置2～8小时仍能至少保持其90%的效价,放置冰箱内则可保持其90%的效价至72小时。稳定性可因葡萄糖、果糖和乳酸的存在而降低,亦随温度升高而降低。供肌内注射可分别溶解125mg、500mg和1g氨苄西林钠地0.9～1.2ml、1.2～1.8ml和2.4～7.4ml灭菌注射用水中。氨苄西林钠静脉滴注液的浓度不宜超过30mg/ml。
不良反应
1.本品不良反应与青霉素相仿,以过敏反应较为常见。皮疹是最常见的反应,多发生于用药后5天,呈荨麻疹或斑丘疹;亦可发生间质性肾炎;过敏性休克偶见,一旦发生,必须就地抢救,予以保持气道畅通、吸氧及给用肾上腺素、糖皮质激素等治疗措施。
2.粒细胞和血小板减少偶见于应用氨苄西林的病人。
3.抗生素相关性肠炎少见,少数病人出现血清转氨酶升高。
4.大剂量氨苄西林静脉给药可发生抽搐等神经系统毒性症状,婴儿应用氨苄西林后可出现颅内压增高,表现为前卤隆起。
禁忌
有青霉素类药物过敏史或青霉素皮肤试验阳性患者禁用。
注意事项
1.应用本品前需详细询问药物过敏史并进行青霉素皮肤试验。
2.传染性单核细胞增多症、巨细胞病毒感染、淋巴细胞白血病、淋巴瘤患者应用本品时易发生皮疹,宜避免使用。
3.本品须新鲜配制。
贮藏
密闭，在凉暗（避光并不超过20℃）干燥处保存。
