适应症
本品主要用于疟疾的预防，也可用于治疗弓形虫病。
用法用量
1、成人常用量、口服。①预防用药，应于进入疫区前1～2周开始服用，一般宜服至离开疫区后6～8周，每周服4片；②耐氯喹虫株所致的恶性疟，每日2片，分2次服，疗程3日；③治疗弓形虫病，每日50～100mg顿服，共1～3日（视耐受力而定），然后每日服25mg，疗程4～6周。
2、小儿常用量、口服。①预防用药，一次按体重0.9mg/kg,每周服1次，最高剂量以成人量为限；②耐氯喹虫株所致的恶性疟，每次按体重0.3mg/kg，一日3次，疗程3日；③弓形虫病，每日按体重1mg/kg，分2次服，服用1～3日后改为每日0.5mg/kg，分2次服，疗程4～6周。
不良反应
口服一般抗疟治疗量时，毒性很低，较为安全。大剂量应用时，如每日用25mg，连服一个月以上，就会出现叶酸缺乏现象。主要影响生长繁殖特别迅速的组织，如骨髓、消化道粘膜，引起造血机能及消化道症状，如味觉的改变或丧失，舌头疼痛、红肿、烧灼感及针刺感，口腔溃疡、白斑等，食道炎所致的吞咽困难、恶心、呕吐、腹痛、腹泻等。较严重的是巨细胞性贫血、白细胞减少症等，如及早停药，能自行恢复。甲酰四氢叶酸可改善骨髓功能。由于过敏所致的皮肤红斑则较少见。
禁忌
（1）妊娠妇女禁用，动物实验证明本品可引起胎仔畸形。（2）哺乳期妇女亦禁用，因本品可由乳汁排出，干扰婴儿的叶酸代谢，若婴儿有葡萄糖-6-磷酸脱氢酶(G-6-PD)缺乏，则可引起溶血性贫血。
注意事项
（1）下列情况应慎用；①意识障碍，大剂量治疗弓形虫病时可引起中枢神经系统毒性反应并可干扰叶酸代谢；②G-6-PD缺乏者，服用本品可能引起溶血性贫血；③巨细胞性贫血患者，服用本品可影响叶酸代谢。（2）大剂量治疗时每周应检测白细胞及血小板2次。
