适应症
本品用于单纯饮食控制不满意的Ⅱ型糖尿病人，尤其是肥胖者和伴高胰岛素血症者，用本品不仅有降血糖作用，还可能有助于减轻体重和高胰岛素血症的效果。对某些经磺酰脲类治疗效果差的糖尿病患者，本品与磺酰脲类降血糖药合用，可产生协同作用，较分别单用的效果更好。
用法用量
剂量应遵医嘱，采用个性化给药原则。 1. 单独治疗给药方法：开始治疗时，一般口服每日一次，每次25mg，餐前服用；数日后，可增加给药次数至2～3次，每次25mg。 2. 与磺酰脲类药物合用时：第一周每天一次，每次25mg，餐前服用；第二周检测血糖后，可逐渐增加每天给药次数至每天2～3次，每次25mg，直至血糖水平降至或接近正常值。本品每天最大口服剂量一般不超过75mg，否则易发生高乳酸血症或乳酸性酸中毒；为了减少胃肠道副反应，本品应与食物同服。
不良反应
1. 常见的有:恶心，呕吐，腹泻，口有金属味。2. 可有乏力，疲倦，体重减轻，头晕，皮疹。3. 亦可发生乳酸酸中毒，临床表现为呕吐，腹痛，过度换气，神志障碍血液中乳酸浓度增加而不能用尿毒症，酮症酸中毒或水杨酸中毒解释。4. 可减少肠道吸收维生素B12，使血红蛋白减少，产生巨幼细胞性贫血，也可引起吸收不良。
禁忌
1. Ⅱ型糖尿病伴有酮症酸中毒、肝及肾功能不全(血清肌酐超过1.5mg/dl)?心力衰竭、急性心肌梗死、严重感染和外伤、重大手术以及临床有低血压和缺氧情况。2. 糖尿病合并严重的慢性并发症(如糖尿病肾病、糖尿病眼底病变)。3. 静脉肾盂造影或动脉造影前。4. 严重心、肺疾病患者。5. 维生素B12?叶酸和铁缺乏的患者。6. 全身情况较差的患者(如营养不良、脱水)。7. 酗酒者。8. 已知对本品过敏者。
注意事项
1. 乳酸性酸中毒：伴有缺氧性疾病(如心衰、呼衰、高血压、肝肾功能减损者)的糖尿病患者，以及服药期间饮酒，伴有严重厌食、呕吐和酮症等糖尿病患者，更易产生乳酸性酸中毒。2. 如果出现严重胃肠道不良反应，应减少本品用量或停用本品。3. 胰岛素依赖型糖尿病不应单独使用本品(可与胰岛素合用)。4. 对胰岛素依赖型及非胰岛素依赖型需要胰岛素治疗的病人，本品与胰岛素联用有协同作用，减少胰岛素的用量，也可能有助于某些不稳定型糖尿病人病情的稳定;加用本品后，须及时减少胰岛素剂量(开始时减少20%～30%)，以防止出现低血糖反应。5. 低血糖反应：单独使用本品时，很少产生低血糖反应。在调整本品剂量期间，特别是本品与胰岛素或磺酰脲类药物联合用药时，可能产生低血糖反应，应小心观察各种症状，避免低血糖反应发生。6. 用药期间要经常检查空腹血糖、尿糖及尿酮体，定期检查糖化血红蛋白以指导医生调整用药剂量，尤其是在联合应用胰岛素以前，必须做血糖和尿糖检查。
