适应症
本品主要用于慢性室性心律失常，如室性早博、室性心动过速。
用法用量
1.口服：首次200～300mg（4-6片），必要时2小时后再服100～200mg（2-4片）。|2.一般维持量每日约400～800mg（8-16片），分2～3次服。成人极量为每日1200mg（24片），分次口服。
不良反应
1.约20%～30%患者口服发生不良反应。|2.胃肠反应:最常见。包括恶心、呕吐等，有肝功能异常的报道，包括GOT增高。|3.神经:为第二位常见不良反应。包括头晕、震颤(最先出现手细颤)?共济失调、眼球震颤、嗜睡、昏迷及惊厥、复视、视物模糊、精神失常、失眠。|4.心血管:窦性心动过缓及窦性停博一般较少发生。偶见胸痛，促心律失常作用如室性心动过速，低血压及心力衰竭加剧。治疗包括停药、用阿托品、升压药、起搏器等。|5.过敏反应:皮疹。|6.极个别有白细胞及血小板减少。
禁忌
心源性休克和有Ⅱ或Ⅲ度房室传导阻滞，病窦综合征者禁用。
注意事项
1.本品在危及生命的心律失常患者中有使心律失常恶化的可能，在程序刺激试验中，此种情况见于10%的患者，但不比其他抗心律失常药高。|2.美西律可用于已安装起博器的Ⅱ度和Ⅲ度房室传导阻滞病人，有临床试验表明在Ⅰ度房室传导阻滞的病人中应用较安全，但要慎用。|3.美西律可引起严重心律失常，多发生于恶性心律失常患者。|4.在低血压和严重充血性心力衰竭病人中慎用。|5.肝功能异常者慎用。|6.室内传导阻滞或严重窦性心动过缓者慎用。|7.用药期间注意随访检查血压、心电图、血药浓度。
