适应症
本品用于预防及治疗血管内血栓栓塞性疾病，施行手术或受伤后血栓性静脉炎、肺栓塞、心肌梗塞及心房纤颤引起的栓塞。适用于需长期维持抗凝者。对急性动脉闭塞，需迅速抗凝时，一般先用肝素控制危状，再用本品维持治疗。遗传性易栓症需长期抗凝者，与肝素合并用药2～5天，再以本品长期维持抗凝。
用法用量
口服给药:|1.第一天2～2次，每次0.1g(2片)，第二天以后每日1～2次，每次0.05g(1片)。根据凝血酶原时间决定增减剂量，凝血酶原时间宜控制在25～30秒(正常值12秒)或INR值2～3。维持量0.05～0.1g(1～2片)，1日1次。极量每次0.3g(6片)。|2.与肝素联合应用可达到满意效果。具体方法是:在24～48小时内静注肝素5000～10000u(分4～8次静注)，或在24小时内静滴肝素15000～20000u， 同时口服双香豆素片，第一次0.2～0.3g(4～6片)，以后用维持量每天0.05～0.1g(1～2片)，并参考凝血酶原时间的数值来调整剂量。
不良反应
过量易致出血，最常见的是无症状的血尿、瘀斑、鼻衄、齿龈出血和咯血。个别病人可出现头昏、恶心、腹泻、皮肤过敏反应，严重持续性头疼、腹疼、背疼。长期服用如突然停药，部分病人于1～2个月内可加重冠状动脉闭塞及栓塞形成。
禁忌
有出血倾向、妊娠、严重肝肾功能不全、严重高血压、活动性消化性溃疡、亚急性感染性心内膜炎等禁用。恶病质、衰弱、发热、活动性肺结核、充血性心力衰竭、月经过多、先兆流产等慎用。
注意事项
本品起效慢，难应急需，在治疗开始1～2天内多与肝素合用。用药过程中定期检查凝血酶原时间。长期口服停药时，要逐渐减量。

密封保存。
