适应症
去极化型骨骼肌松弛药。可用于全身麻醉时气管插管和术中维持肌松。
用法用量
本品必须在具备辅助或控制呼吸的条件下使用：1．气管插管时，1～1.5mg/kg，最高2mg/kg；小儿1～2mg/kg，用0.9%氯化钠注射液稀释到每ml含10mg，静脉或深部肌内注射，肌内注射一次不可超过150mg。2．维持肌松：一次150～300mg溶于500ml5%～10%葡萄糖注射液或1%盐酸普鲁卡因注射液混合溶液中静脉滴注。
不良反应
1．高血钾症本品引起肌纤维去极化时使细胞内K+迅速流至细胞外。正常人血钾上升0.2～0.5mmol/L；严重烧伤、软组织损伤、腹腔内感染、破伤风、截瘫及偏瘫等，在本品作用下引起异常的大量K+外流致高血钾症，产生严重室性心律失常甚至心搏停止。2．心脏作用本品的拟乙酰胆碱作用可引起心动过缓、结性心律失常和心搏骤停，尤其是重复大剂量给药最易发生。3．眼内压升高本品对眼外肌引起痉挛性收缩以致眼压升高。4．胃内压升高最高可达40cmH2O，并可引起饱胃病人胃内容反流误吸。5．恶性高热多见于本品与氟烷合用的病人；也多发生于小儿。6．术后肌痛给药后卧床休息者肌痛轻而少，1～2天内即起床活动者肌痛剧而多。7．可能导致肌张力增强以胸大肌最为明显，其次是腹肌，严重时波及肱二头肌和股四头肌等。这时不仅机体总的氧耗量加大，足以引起胃内压甚至颅内压升高。
禁忌
1．脑出血、青光眼、视网膜剥离、白内障摘除术、低血浆胆碱酯酶、严重创伤大面积烧伤、上运动神经元损伤的病人及高钾血症患者禁用。
2．使用抗胆碱酯酶药者慎用。
注意事项
1．不具备控制或辅助呼吸条件时，严禁使用。2．忌在病人清醒下给药。3．严重肝功能不全、营养不良、晚期癌症、严重贫血、年老体弱、严重电解质紊乱等患者慎用。4．接触有机农药患者，已证明无血浆胆碱酯酶减少或抑制者，方能使用至足量。5．为了解除本品肌松作用引起的短暂纤维颤动，可预先静脉注射小剂量非去极化肌松药(维库溴铵0.5mg)。6．预先给予阿托品可防止本品对心脏的作用。7．出现长时间呼吸停止，必须用人工呼吸，亦可输血，注射干血浆或其他拟胆碱酯酶药，但不可用新斯的明。
