适应症
本品适用于治疗下列肿瘤：难治性睾丸肿瘤――经合适手术，化疗及放疗后的难治性睾丸肿瘤患者，可用本品与其他获准使用的化疗药物联合治疗。小细胞肺癌――注射用磷酸依托泊苷联合其他获准使用的化疗药物作为小细胞肺癌患者的一线药物。
用法用量
1.依托泊苷合并其他认可的化疗药治疗睾丸癌患者的常用剂量为50～100mg/m2/日，连续5天，或50～100mg/m2/日，第1、3、5天给药。应据此使用磷酸依托泊苷等效剂量。 2.依托泊苷与其他认可的化疗药合并用于小细胞肺癌的常用剂量是从35mg/m2/日连续4天至50mg/m2/日连续5天，磷酸依托泊苷则应用等效剂量。3.本品为供静脉输注的制剂，输注速率为5～210分钟，使用前先用5ml或10ml无菌注射用水、5%葡萄糖溶液、生理盐水、苯甲醇抑菌注射液或苯甲醇抑菌注射用氯化钠溶液在包装小瓶内溶解为相当于依托泊苷20mg/ml或10mg/ml（即磷酸依托泊苷22.7mg/ml或11.4mg/ml）的溶液备用，该溶液可直接使用或进一步用5%葡萄糖溶液、生理盐水稀释使用，最低浓度为0.1mg/ml的依托泊苷（即0.114mg/ml的磷酸依托泊苷）。
不良反应
最常见的临床不良反应是白细胞和中性粒细胞减少:1. 血液系统的毒性:磷酸依托泊苷的骨髓抑制效应呈剂量相关性，对白细胞的抑制效应在用药后第15～22天达峰，对中性粒细胞的抑制效应在用药后第12～19天达峰，对血小板的抑制效应在用药后第10～15天达峰。骨髓相通常于用药后第21天恢复正常，尚未见药物蓄积中毒的报道。2. 消化系统的毒性:主要表现为恶心、呕吐、一般为轻到中度，常用止吐方法即可控制该不良反应。3. 循环系统的毒性:临床研究中，静脉输注磷酸依托泊苷可使患者血压升高或降低，治疗期间若发生有临床意义的血压升高或降低可采用适当的对症治疗方法。4. 过敏反应:约有3%的患者在输注药物过程中可发生过敏反应，表现为畏寒、寒战、心动过速、支气管痉挛、呼吸困难、发热、出汗、皮肤瘙痒、血压升高或下降、意识丧失、恶心呕吐。停止输注药物或采用对症支持治疗通常可缓解，但该不良反应也可为致死性的。初次输注磷酸依托泊苷即可发生类过敏反应，出现面部/舌肿胀、咳嗽、出汗、紫绀、喉头紧迫感、喉头痉挛、背痛和/或意识丧失，出现呼吸暂停的报道罕见。5. 脱发:44%的患者可出现可逆性脱发，偶可出现全秃。6. 皮肤毒性:在临床剂量下偶可出现皮疹、荨麻疹和/或皮肤瘙痒等不良反应。7. 其他毒性反应:少数患者可出现味觉异常、发热、色素沉着、腹痛、便秘、吞咽困难、一过性皮质盲和神经炎等不良反应，中毒性肝炎罕见。
禁忌
凡既往对磷酸依托泊苷、依托泊苷和本品中任一组分过敏的患者禁用。
注意事项
1. 本品的多数不良反应如及时发现. 及时治疗均是可逆性的，严重不良反应发生时应立即减量或停药，并视病情采取相应治疗措施，重新用药前应权衡利弊，严密监测，高度警惕可能发生的不良反应。2. 血浆蛋白水平低下的患者更易发生不良反应。3. 每个疗程开始前应进行血液学检查，血小板计数
