适应症
用于治疗难治性精神分裂症。
用法用量
口服，取出药片，立即放入口中。不需用水，药物便迅速崩解随唾液咽下。 1.初始治疗： (1)建议从12.5mg开始，一天1次或2次。应以每日增加25～50mg剂量的方法继续给药治疗，如果患者耐受性好，可在第2周末将剂量增加至常用治疗剂量200～400mg/日，分三次给药。 (2)此后，每周增加剂量的次数不超过1次或2次，每次增加的剂量不超过100mg，最高剂量可达600mg/日。 (3)低剂量给药时，可将本品掰开给药，剩下的半片药品应丢弃。为了将低血压、癫痫和镇静的危险性降至最低，谨慎滴定剂量并分次给药是必要的。 2.维持治疗： (1)氯氮平对精神分裂症维持治疗的疗效还在研究中，但其他抗精神病药维持治疗的疗效已证明是确切的。 (2)建议治疗有效的患者继续服用本品，并以最小剂量维持治疗。 (3)由于氯氮平的严重不良反应，患者须定期检查，以进一步确定治疗方案。维持量为一日100～200mg。 3.停止治疗：停止治疗时建议在1～2周内逐步减小剂量。但如患者的身体状况需立即停药（如白细胞减少），应对患者进行密切观察，以防精神分裂症状复发和胆碱能作用反跳所致症状如头痛、恶心、呕吐和腹泻的发生。
不良反应
1.镇静作用强和抗胆碱能不良反应较多，常见有头晕、无力、嗜睡、多汗、流涎、恶心、呕吐、口干、便秘、体位性低血压、心动过速。
2.常见食欲增加和体重增加。
3.可引起心电图异常改变。可引起脑电图改变或癫痫发作。
4.也可引起血糖增高。
5.严重不良反应为粒细胞缺乏症及继发性感染。
禁忌
1.严重心、肝、肾疾患、昏迷、谵妄、低血压、癫痫、青光眼、骨髓抑制或白细胞减少者禁用。
2.对本品过敏者禁用。
注意事项
1.出现过敏性皮疹及恶性综合征应立即停药并进行相应的处理。
2.中枢神经抑制状态者慎用。尿潴留患者慎用。
3.治疗头3个月内应坚持每1-2周检查白细胞计数及分类，以后定期检查。
4.定期检查肝功能与心电图。
5.定期检查血糖，避免发生糖尿病或酮症酸中毒。
6.用药期间不宜驾驶车辆、操作机械或高空作业。
7.用药期间出现不明原因发热，应暂停用药。

密封，置阴凉处。
