适应症
适用于慢性阻塞性肺病（COPD）的维持治疗，包括慢性支气管炎和肺气肿，伴随性呼吸困难的维持治疗及急性发作的预防。
用法用量
临用前，取胶囊1粒放入专用吸入器的刺孔槽内，用手指揿压按钮，胶囊两端分别被细针刺孔，然后将口吸器放入口腔深部，用力吸气，胶囊随着气流产生快速旋转，胶囊中的药粉即喷出囊壳，并随气流进入呼吸道。成人：一次1粒，一日1次。对老年患者、肝功能不全和肾功能不全患者无需调整剂量，但对中重度肾功能不全患者（CLCR＜50ml/min）必须进行密切监控。
不良反应
尚不明确。
禁忌
1、禁用于对噻托溴铵或本品所含有其它成分如乳糖过敏者。|2、禁用于对阿托品或阿托品衍生物过敏者。
注意事项
1、噻托溴铵作为每日一次维持治疗的支气管扩张药，不应用作支气管痉挛急性发作的初始治疗，即抢救治疗药物。|2、在吸入噻托溴铵粉末后有可能立即发生过敏反应。|3、与其他抗胆碱的药物一样，对于窄角型青光眼、前列腺增生、或膀胱颈梗阻的患者应谨慎使用。|4、吸入药物可能引起吸入性支气管痉挛。|5、与所有主要经肾脏排泄的药物一样，对于中、重度肾功能不全（肌酐清除率≤50ml/min）的患者，只有在预期利益大于可能产生的危害时，才能使用噻托溴铵。尚无严重肾功能不全患者长期使用噻托溴铵的经验。|6、胶囊应该密封于囊泡中保存，仅在用药时取出，取出后应尽快使用，否则药效会降低，不小心暴露于空气中的胶囊应丢弃。|7、患者需注意避免将药物粉末弄入眼内。必须告知患者药粉误入眼内可能引起或加重窄角型青光眼、眼睛疼痛或不适、短暂视力模糊、视觉晕轮或彩色影像并伴有结膜充血引起的红眼和角膜水肿的症状。如果出现窄角型青光眼的征象，应停止使用噻托溴铵并立即去看医生。|8、口干，是由抗胆碱能治疗引起的，长期可引起龋齿。|9、噻托溴铵的使用不得超过一天一次。|10、本胶囊仅供吸入，不能口服。|11、未进行对驾驶和操作机器能力影响的研究。根据在推荐剂量下得到的药理学和不良反应特性，未有证据显示会影响驾驶和操作机器的能力。
