适应症
主要用作叶酸拮抗剂（如甲氨蝶呤、乙胺嘧啶或甲氧苄啶等）的解毒剂。本品临床常用于预防甲氨蝶呤过量或大剂量治疗后所引起的严重毒性作用。当口服叶酸疗效不佳时，也用于口炎性腹泻、营养不良、妊娠期或婴儿期引起的巨幼细胞性贫血，但对维生素B12缺乏性贫血并不适用。近年应用亚叶酸钙作为结肠、直肠癌的辅助治疗，与氟尿嘧啶联合应用，可延长存活期。
用法用量
1、高剂量甲氨蝶呤治疗后亚叶酸钙“解救”疗法：根据甲氨蝶呤的血药浓度决定亚叶酸钙的剂量。一般静脉注射甲氨蝶呤24小时后，给予亚叶酸钙10mg/m2，每6时1次，共10次。如有消化道毒性反应出现时，不要鞘内注射本品。亚叶酸钙使用指导剂量：2、甲氨蝶呤消除不畅或不慎超剂量使用时：当不慎超剂量使用甲氨蝶呤时，应尽可能及时地使用亚叶酸钙进行急救；排泄延迟时，也应在甲氨喋呤使用24小时内应用亚叶酸钙。一般每6小时肌注或静脉注射亚叶酸钙10mg，直到血中甲氨蝶呤水平低于10-8M（0.01μmol）。出现消化系统反应（如恶心、呕吐）时，亚叶酸钙可胃肠外给药，但不可鞘内注射。血肌酐、甲氨喋呤水平每隔24小时测定一次，如24小时血肌酐增加超过50%或24小时甲氨蝶呤超过9×10-7M，亚叶酸钙剂量增加到100mg/m2，每3小时一次，直到甲氨蝶呤水平低于10-8M。3、叶酸缺乏引起的巨细胞性贫血：一般每天1mg，尚无根据证明剂量增加疗效会增加。4、结直肠癌：（1）5-Fu370mg/m2静注后，亚叶酸钙200mg/m2，缓慢静脉注射，不少于3分钟。（2）5-Fu425mg/m2静注后，亚叶酸钙20mg/m2，静脉注射。
不良反应
很少见，偶见皮疹、荨麻疹或引起哮喘急性发作等。
禁忌
尚不明确。
贮藏
遮光，密闭保存。
