适应症
主要用作叶酸拮抗剂（如甲氨蝶呤、乙胺嘧啶或甲氧苄啶等）的解毒剂。本品临床常用于预防甲氨蝶呤过量或大剂量治疗后所引起的严重毒性作用。当口服叶酸疗效不佳时，也用于口炎性腹泻、营养不良、妊娠期或婴儿期引起的巨幼细胞性贫血，但对维生素B12缺乏性贫血并不适用。近年应用亚叶酸钙作为结肠、直肠癌的辅助治疗，与氟尿嘧啶联合应用，可延长存活期。
用法用量
规定的剂量和给药时间均应严格遵守，不得随意改变。补加剂量或停药必须经负责医师同意。肌内注射作为甲氨蝶呤的“解救”疗法，本品剂量最好根据血药浓度测定。一般采用剂量按体表面积为9～15mg/m2,每6～8小时一次，持续2日，直至甲氨蝶呤血清浓度在5×10-8mol/L以下。作为乙胺嘧啶或甲氧苄啶等的解毒剂，每次剂量肌注9～15mg，视中毒情况而定。用于贫血，每日肌注1mg。静脉注射作为结肠-直肠癌的辅助治疗，与氟尿嘧啶联合应用;本品静脉注射200mg/m2，注射时间不少于3分钟，接着用氟尿嘧啶300～400m
不良反应
很少见，偶见皮疹、荨麻疹或引起哮喘急性发作等。
