适应症
1.高胆固醇血症:在高胆固醇血症和高甘油三酯血症并存而以高胆固醇血症为主的患者，辛伐他汀可降低胆固醇水平。|2.冠心病对于冠心病合并高胆固醇血症的患者，辛伐他汀适用于:减少死亡的危险性;减少冠心病死亡及非致死性心肌梗塞的危险性;降低因冠脉事件需要进行心脏血管再通手术(冠状动脉搭桥术及经皮冠状动脉腔内成形术)的几率;延缓动脉粥样硬化的进程，包括减少新病灶及全堵塞的发生。
用法用量
1.高胆固醇血症:一般起始剂量为每天10mg，晚间顿服。对于胆固醇水平轻至中度升高的患者，起始剂量为每天5mg。若需调整剂量，应间隔四周以上，最大剂量为每天40mg，晚间顿服。|2.冠心病:冠心病患者可以20mg/天为起始剂量，如需要剂量调整，应间隔四周以上，最大剂量为每天40mg，晚间顿服。 |3.合并用药:辛伐他汀单独应用或与胆酸螯合剂协同应用时均有效。一般情况下应避免与贝特类或烟酸类药物同时应用。同时服用免疫抑制剂（如环孢菌素）的患者，辛伐他汀的起始剂量应为5mg/天，且不超过10mg/天。 |4.肾功能不全的病人:中度肾功能不全的病人不必调整剂量。严重肾功能不全(肌酐清除率＜30ml/分)的患者应慎用本品，此类病人的起始剂量应为5mg/天，当剂量超过10mg/天时，应严密监测。
不良反应
1.辛伐他汀一般耐受性良好，大部分不良反应轻微且为一过性。在临床对照研究中不足2%的病人因辛伐他汀的不良反应而中途停药。|2.在临床对照研究中，与药物有关 的发生率≥1％的不良反应有腹痛、便秘、胃肠胀气，发生率在0.5%～0.9%的不良反应有疲乏无力、头痛。肌病的报告很罕见。|3.实验室检查发现：血清转氨酶显著和持续升高的情况罕有报道。肝功能检查异常一般为轻微或一过性的。
禁忌
1.对本品任何成分过敏者。|2.动性肝炎或无法解释的持续血清转氨酶持续升高者。|3.怀孕及哺乳期妇女。|4.禁止与四氢酚类钙通道阻滞剂米贝地尔合用。
注意事项
1.肌肉作用:HMG-CoA还原酶抑制剂偶尔会引起肌病，表现为肌肉疼痛或无力并伴有CK显著升高（高于正常值上限的10倍）。伴有或不伴有继发性肌红蛋白尿症的急性肾功能衰竭的横纹肌溶解罕见报道。药物相互作用引起的肌病 HMG-CoA还原酶抑制剂与单独用药即可引起肌病的药物合并使用时，会增加肌病的发生率和严重程度，这些药物包括吉非贝齐和其它贝特类，以及降脂剂量 （≥1g/天）的烟酸。辛伐他汀与CYP3A4的强抑制剂包括环孢菌素、米贝地尔、伊曲康唑、酮康唑、红霉素、克拉霉素?HIV蛋白酶抑制剂及奈法唑酮合 用，可能增加肌病的危险。|2.肝脏作用临床研究中，少数接受辛伐他汀治疗的成年患者出现持续的血清转氨酶显著升高（高于正常值上限的3倍）。这些患者间断或终止用药后，转氨酶水平通常缓慢地降低至治疗前水平。|3.高甘油三酯血症辛伐他汀仅有中等程度降低甘油三酯的作用，不适合治疗以甘油三酯升高为主的异常情况（如Ⅰ?Ⅳ及Ⅴ型高脂血症）。|4.大量饮酒和/或有肝脏病史的患者，应慎用本品。
