适应症
适用于12岁以上患者，用于治疗下列敏感菌引起的轻、中度感染：|1.慢性支气管炎急性发作：由流感嗜血杆菌、卡他莫拉菌、肺炎链球菌引起。|2.急性支气管炎：由卡他莫拉菌、肺炎链球菌引起。|3.社区获得性肺炎：由嗜肺军团菌、肺炎支原体、肺炎链球菌引起。|4.咽炎和扁桃体炎：由化脓性链球菌引起。|5.单纯性皮肤和软组织感染：由金黄色葡萄球菌(甲氧西林敏感菌株)、化脓性链球菌引起。|注：青霉素是治疗和预防链球菌感染(包括预防风湿热)的常用药，一般说来地红霉素在治疗化脓性链球菌引起的鼻咽炎时有效，并能根除;对预防风湿热的疗效尚无报道。
用法用量
本品应与食物同服或饭后1小时内服用，不得分割、压碎、咀嚼。|1.慢性支气管炎急性发作0.5g一天，一次5-7。|2.急性支气管炎0.5g一天，一次7。|3.社区获得性肺炎0.5g一天，一次14。|4.咽炎和扁桃体炎0.5g天，一次10。|5.单纯性皮肤和软组织感染0.5g，一天一次5-7。
不良反应
1.3299例患者口服本(0.5g/天)7-14天，没有发现与毒性有关的死亡或致残。87例患者(2.6%)因不良反应终止用药，其中35例(40%)是因恶心和腹痛终止用药。|2.另一临床试验(口服本药0.5g/天，疗程5天)结果表明，35例患者(3.8%)因不良反应止用药，其中15例(43%)是因恶心和腹痛终止用药。|3.与本药治疗有关的不良反应如下：头痛(7.7%)、腹痛(7.1%)、腹泻(6.7%)、恶心(5.9%)、消化不良(2.6%)、眩晕/头昏(2.1%)、皮疹(1.4%)、呕吐(1.2%)等。
禁忌
1.对地红霉素和其他大环内酯类药物过敏的患者禁用。|2.不应用于可疑或潜在菌血症患者(因其不能提供有效的药物浓度达到治疗部位)。
注意事项
1.肝功能不全：轻度肝损伤患者，其平均Cmax.AUC和分布体积随服药次数的增多而略有增加，但不必调整剂量。|2.肾功能不全：其平均Cmax.AUC随肌酐清除率的降低而趋于升高，但肾脏损伤(包括透析)患者不必调整剂量。|3.已有报道，实际上使用所有的广谱抗生素药(包括地红霉素)，都会产生伪膜性结肠炎。因此，若使用抗生素的患者发生腹泻，考虑到这种诊断是重要的。这种结肠炎的程度从轻度至危及生命，程度不同。对于轻度伪膜性结肠炎病例，通常仅仅是停药就能奏效，对于中度至严重病例，就应采取适当的治疗措施。
