适应症
本品适用于敏感菌所致的呼吸道感染、消化道感染、泌尿道感染、皮肤软组织感染、骨和关节感染、口腔及耳鼻喉感染等。
用法用量
静脉滴注。先用0.9%氯化钠注射液稀释，并在4小时内用完。成人：常规剂量为每日4～6g，分次静脉滴注。病情严重时可增加剂量，每日最大量为12g。儿童：常用量常规剂量为每日50～200mg/kg，分次静脉滴注。
不良反应
1.过敏反应:较为常见，包括皮疹、药物热、哮喘等，偶可能引起过敏性休克。|2.消化道反应:包括恶心、呕吐、腹泻等反应，偶见伪膜性结肠炎。|3.肝毒性:少数患者用药后出现ALT?AST增高，也有急性肝脏胆汁淤积的报道。|4.肾毒性:偶有急性间质性肾炎的报道。|5.神经系统反应:大剂量静脉注射氟氯西林可引起头痛、抽搐、惊厥等神经系统反应，此反应尤易见于肾功能衰退患者。偶有兴奋、焦虑或失眠的报道。|6.血液系统反应:偶有中性粒细胞减少、嗜酸粒细胞增多、血小板减少或溶血性贫血的报道。|7.长期:大剂量用药可致菌群失调，出现由念珠菌或耐药菌引起的二重感染。
禁忌
1.对本药任一成分或其他青霉素类药物过敏者。|2.传染性单核细胞增多症、巨细胞病毒感染、淋巴细胞性白血病、淋巴瘤等患者。
注意事项
1.交叉过敏:对一种青霉素类药过敏者可能对其他青霉素类药过敏,也可能对青霉素胺或头孢菌素类药过敏因此使用前必须详细询问青霉素过敏史、其他药物过敏史及过敏性疾病史如发生严重的过敏反应需立即用肾上腺素治疗,进行吸氧、静脉注射类固醇、喉管导气处理。|2.有哮喘、湿疹、枯草热、荨麻疹等过敏性疾病史者慎用。|3.肝、肾功能严重损害者慎用|4.本药与氨基糖苷类药(如庆大霉素、卡那霉素)?环丙沙星、培氟沙星等药呈配伍禁忌,联用时不可置于同一容器内。|5.以硫酸铜法进行尿糖测定时可呈假阳性,用葡萄糖法测定则不受影响。|6.用药时,可用10ml静脉注射用水作为本药的稀释液。在粉末溶解时含药溶液会显示出一过性粉红色,但在5分钟内溶液会变成淡黄色,此种情况为正常现象。
