适应症

口腔用局部麻醉剂，特别适用于涉及切骨术及粘膜切开的外科手术过程。

用法用量

局部浸润或神经阻滞麻醉，口腔内粘膜下注射给药。注射前请抽回血以检查是否误入血管，尤其行神经阻滞麻醉时。注射速度不得超过1ml/分钟。|1.成人：必须根据手术需要注射适当的剂量。对于一般性手术，通常给药剂量为1/2~1支或遵医嘱。盐酸阿替卡因最大用量不得超过7mg/kg体重。|2.4岁以上儿童：必须根据儿童的年龄、体重、手术类型使用不同的剂量。盐酸阿替卡因最大用量不超过5mg/kg体重。|3.盐酸阿替卡因的儿童平均使用剂量以mg计可计算如下：儿童的体重（kg）×1.33。|4.老年人：使用成人剂量的一半。

不良反应

如同所有的口腔麻醉剂，使用本品病人有可能出现晕厥。本品含有的焦亚硫酸盐可能引起过敏反应或加重过敏反应。用药过量或某些敏感的患者可能出现以下临床症状或体征：|1.中枢神经系统：神经质、激动不安、呵欠、震颤、忧虑、眼球震颤、多语症、头痛、恶心、耳鸣。如出现以上症状，应要求患者过度呼吸，严密监视以防中枢神经抑制造成病情恶化伴发癫痫。|2.呼吸系统：呼吸急促，然后呼吸过缓，可能导致呼吸暂停。|3.心血管系统：心动过速、心动过缓、心血管抑制伴随动脉低血压，可能导致虚脱、心律失常（室性早博、室颤）、传导阻滞（房室阻滞）。以上心脏表现可能导致心脏停博。

禁忌

下列患者禁用本品：|1.4岁以下儿童；|2.对局麻药或本品中其他成分过敏者；|3.严重房室传导障碍而无起搏器的患者；|4.经治疗没有得到控制的癫痫患者；|5.卟啉病。|6.本品不建议与西布曲明合用。

注意事项

1.必须进行临床综合考虑，了解病史，以便决定手术方法、同步治疗方案。|2.应先行注射5％－10％的剂量，试验是否存在过敏反应。|3.本品应缓慢注射，严禁注射于血管中，注射前必须做抽回血检查。|4.使用本品的过程中应维持与患者的语言交流。|5.接受抗凝剂治疗者须严密监视（监测国际标准化比值）。|6.高血压或糖尿病患者慎用本品。|7.本品可能引起局部组织坏死。|8.麻醉咬合危险：各种咬合（唇、颊、粘膜、舌），建议患者在感觉恢复前不要咀嚼口香糖或食物。|9.避免注射于感染及炎症部位（麻醉效果降低）。|10.运动员使用需注意本药活性成分可引起兴奋剂检查尿检结果出现阳性。|11.因酰胺类局麻药主要由肝脏代谢，严重肝功能不全患者需降低剂量。缺氧、高钾血症、代谢性酸中毒患者亦需降低使用剂量。|12.本品中含有10万分之一的肾上腺素，以下情况须严密注意：|(1)除心动过缓之外的各种类型的心律失常；|(2)冠状动脉供血不足；|(3)严重动脉高血压与一些药物（见药物相互作用）合并应用时，需严密监测患者的临床症状以及生化指标。
