适应症

用于治疗全身性或部分性癫痫，尤其是以下的发作类型：失神发作、肌阵挛发作、失张力发作和混合性发作以及部分性癫痫：简单性或复杂性发作；继发性全身性发作；特殊类型的综合症（West,Lennox-Gastaut）。

用法用量

1.口服 1成人常用量：每日按体重15mg/kg或每日600～1200mg，分次服。开始时按体重5～10mg/kg，一周后递增，至发作得以控制为止。当每日用量超过250mg时，每日应分次服用，以减少胃肠道刺激。最大量一般为每日按体重不超过30mg/kg，或每日1.8～2.4g。
2.小儿常用量：按体重计与成人相同。也有报道开始时按体重15mg/kg，按需要每隔一周增加5～10mg/kg至有效或不能耐受为止。

不良反应

1.少见的有:
(1)过敏性皮疹;
(2)血小板减少症或血小板凝聚抑制以致异常出血或瘀斑;
(3)肝脏中毒出现眼和皮肤黄染。
2.下列的反应如果持续出现时应予注意:
(1)较常见的有：腹泻、消化不良、恶心或呕吐、胃肠道痉挛、月经周期改变;
(2)较少见或罕见的有：便秘、倦睡、一般轻而短暂的脱发、眩晕、疲乏头痛、共济失调、异常地兴奋、不安和烦燥。

禁忌

急性肝炎慢性肝炎个人或家族有严重肝炎史，特别是药物所致肝炎对丙戊酸钠过敏者卟啉症

注意事项

1.于进餐后立即服用，以减少药物对胃部的刺激。
2.对诊断的干扰：尿酮试验可能由于酮性代谢产物随尿排出，出现假阳性。甲状腺功能试验可能受影响。乳酸脱氢酶、丙氨酸氨基转移酶、门冬氨酸氨基转移酶可能轻度升高，并提示无症状性肝脏中毒。血清胆红素可能升高，可提示潜在的严重肝脏中毒。
3.有肝病或明显肝功能损害时禁用。有血液病、肝病史、肾功能损害、器质性脑病时慎用。
4.用药前和用药期间应作血细胞包括血小板计数，肝、肾功能检查，肝功能在最初半年内最好每1～2月复查一次，半年后复查间隔酌量延长。
5.如拟停药，应逐渐减量，以防再次发作；当取代其他抗惊厥药物时，丙戊酸(钠)用量应逐渐增加，而被取代的药物应逐渐减少，以维持对发作的控制。
6.外科手术或其他急症治疗时应考虑可能遇到的出血时间延长或中枢神经抑制药作用的增强。
