适应症
本品为强效镇痛药，适用于其他镇痛药无效的急性锐痛，如严重创伤、战伤、烧伤、晚期癌症等疼痛。心肌梗死而血压尚正常者，应用本品可使病人镇静，并减轻心脏负担。应用于心源性哮喘可使肺水肿症状暂时有所缓解。麻醉和手术前给药可保持病人宁静进入嘈睡。因本品对平滑肌的兴奋作用较强，故不能单独用于内脏绞痛(如胆绞痛等)，而应与阿托品等有效的解痉药合用。本品不适宜慢性重度癌痛病人的长期使用。
用法用量
(1)皮下注射。成人常用量：一次5-15mg，一日10-40mg；极量：一次20mg，一日60mg。 (2)静脉注射。成人镇痛时常用量5-10mg：用作静脉全麻按体重不得超过1mg/kg，不够时加用作用时效短的本类镇痛药，以免苏醒迟延，术后发生血压下降和长时间呼吸抑制。 (3)手术后镇痛注入硬膜外间隙，成人自腰脊部位注入，一次极限5mg，胸脊部位应减为2-3mg。按一定的间隔可重复给药多次。注入蛛网膜下腔，一次0.1-0.3mg。原则上不再重复给药。 (4)对于重度癌痛病人，首次剂量范围较大，每日3-6次，以预防癌痛发生及充分缓解癌痛。
不良反应
尚不明确。
禁忌
呼吸抑制已显示紫绀、颅内压增高和颅脑损伤、支气管哮喘、肺源性心脏病代偿失调、甲状腺功能减退、皮质功能不全、前列腙肥大、捧尿困难及严重肝功能不全、休克尚未纠正控制前、炎性肠梗等病人禁用。
