适应症
本品在治疗确定由叶酸缺乏所致的巨幼细胞贫血方面有良好效果。此疾病主要见于婴幼儿，妊娠期间，吸收不良综合征，肝病，口炎性腹泻和营养不良。对于某些疾病，亚叶酸钙的疗效并不优于叶酸。在减少叶酸拮抗剂毒性或在必要时阻止其起效方面，亚叶酸钙也有良好效果。
用法用量
1? 用于5-Fu合用增效，每次20~500mg/m2，静滴，每日1次，连用5天。可用生理盐水或葡萄糖注射液稀释配成输注液，配制后的输注液pH不得少于6.5。输注液须新鲜配制。 2? 作为甲氨喋呤的“解救”疗法，本品剂量最好根据血药浓度测定。一般采用的剂量为按体表面积9mg~15mg/m2，肌注或静注，每6小时1次，共用12次；作为乙胺嘧啶或甲氧苄啶等的解毒剂，每次剂量为肌注9mg~15mg，视中毒情况而定。
不良反应
大剂量给药可见胃部不适感;偶尔有变态反应，注射给药后病人发热。
禁忌
对本品过敏者禁用;恶性贫血和因维生素B12缺乏时引起的贫血者禁用。
注意事项
1. 亚叶酸钙只能与叶酸拮抗剂(如甲氨蝶呤或氟脲嘧啶类，如5-氟尿嘧啶)合并使用，并且应在对肿瘤化疗药物使用有经验的医师指导下使用。2. 如果患者可能会出现呕吐或不吸收亚叶酸钙时，建议胃肠道外用药。3. 由于亚叶酸钙注射液含有钙离子，静脉注射时每分钟不超过160mg。4. 亚叶酸钙可能会加强氟尿嘧啶的毒性。曾报道有同时接受亚叶酸钙和氟脲嘧啶治疗的老年患者，因严重小肠结肠炎、腹泻和脱水而死亡。5. 有些患者会伴随出现粒细胞缺乏和发热，但并非所有患者都出现。6. 接受亚叶酸钙治疗的肿瘤患者中罕有癫痫和/或晕厥的出现，通常见于同时使用氟脲嘧啶类药物，而且绝大多数为有中枢转移的病例。由于有3例病例再次使用亚叶酸钙后神经系统症状复发，这种情况下不建议继续使用亚叶酸钙。7. 不建议同时使用叶酸拮抗剂和亚叶酸钙，因为叶酸拮抗剂的效果会被减弱或完全抑制。亚叶酸钙对于甲氨蝶呤的非血液系统毒性无对抗作用，例如由药物和/或代谢产物沉积所致的肾脏毒性。8. 甲氨蝶呤不适于治疗恶性贫血和由于维生素B12缺乏所致的贫血。血液系统症状可能会减轻，但神经系统症状会加重。
