适应症
本品用于饮食控制、降低体重及运动锻炼不能有效控制高血糖的2型糖尿病（非胰岛素依赖型）患者。瑞格列奈片可与二甲双胍合用。与各自单独使用相比，二者合用对控制血糖有协同作用。
用法用量
1.瑞格列奈片应在主餐前服用(即餐前服用)。在口服本品30分钟内即出现促胰岛素分泌反应。通常在餐前15分钟内服用本药，服药时间也可掌握在餐前0-30分钟内。|2.请遵医嘱服用瑞格列奈片。剂量因人而异，以个人血糖而定。推荐起始剂量为0.5mg，以后如需要可每周或每两周作调整。接受其他口服降血糖药治疗的患者转用瑞格列奈片治疗的推荐起始剂量为1mg。|3.最大的推荐单次剂量为4mg，进餐时服用。但最大日剂量不应超过16mg。|4.当通常饮食能很好控制血糖的2型糖尿病患者出现暂时的控制失败时，短期使用瑞格列奈可有效控制血糖。|5.对于衰弱和营养不良的患者，应谨慎调整剂量。如果与二甲双胍合用，应减少瑞格列奈片的剂量。尽管瑞格列奈主要由胆汁排泄，但肾功能不全的患者仍应慎用。
不良反应
同其他口服降糖药一样，服用瑞格列奈可能引起血糖变化，如高血糖和低血糖。同每种糖尿病治疗一样，这些反应的出现依赖于个体因素，如饮食习惯，剂量，运动和应激反应。 瑞格列奈及其他降血糖药物的临床应用显示，服用瑞格列奈可能发生以下不良反应： 根据不良反应的发生率分别定义如下： 罕见不良反应：发生率1/10000，1/1000。非常罕见不良反应：发生率1/10000。 |1.免疫系统失调: 过敏反应 可发生皮肤过敏反应，如搔痒、发红、荨麻疹。非常罕见发生广泛的过敏性反应或免疫反应如脉管炎。|2.代谢及营养失调: 高血糖 。
禁忌
1.已知对瑞格列奈或瑞格列奈中的任何赋型剂过敏的患者。|2.1型糖尿病患者(胰岛素依赖型-IDDM，C-肽阴性糖尿病患者)。|3.伴随或不伴昏迷的糖尿病酮症酸中毒患者。
注意事项
1.同其它大多数口服促胰岛素分泌降血糖药物一样，瑞格列奈片也可致低血糖。|2.与二甲双胍合用会增加发生低血糖的危险性。如果合并用药后仍发生持续高血糖，则不能再用口服降血糖药控制血糖，而需改用胰岛素治疗。|3.在发生应激反应时，如发烧、外伤、感染或手术，可能会出现血糖控制失败。这时，有必要停止服用瑞格列奈而进行短期的胰岛素治疗。|4.瑞格列奈片尚未在18岁以下或75岁以上的患者中进行过研究。|5.应尽量避免将瑞格列奈与吉非贝齐合用。如果必须合用，应严密监测患者的血糖水平。因为可能需要减少瑞格列奈的用药剂量。|6.与其它口服降血糖药一样，患者必须慎用以避免开车时发生低血糖。|7.肾功能不全、虽然瑞格列奈水平与肌酐清除率仅有微弱的联系，但本品的总血浆清除率在严重肾功能损伤的患者中略有降低。由于肾功能损伤的糖尿病患者对胰岛素敏感性增强，这些患者增加剂量时应谨慎。|8.肝功能不全、在通常剂量下，与肝功能正常的患者相比，肝功能损伤患者可能暴露于较高浓度的瑞格列奈及其代谢产物。因此肝功能损伤患者应慎用本品。应延长剂量调整间期，对患者的反应进行充分评估。
