适应症
1.本品治疗和预防心绞痛发作。 2.可用于治疗洋地黄类或利尿剂效果不满意的心功能不全。
用法用量
宜自小剂量开始，逐渐增量。将乳膏按刻度挤出所需长度，均匀涂布于所给印有刻度的纸上，每格相当硝酸异山梨酯0.2g，将纸面涂药区全部涂满，即5cm × 5cm 面积，贴在左胸前区(可用胶布固定)一日1次(必要时8小时一次)可睡前贴用。
不良反应
1.头痛，初用时易发生。
2.低血压，伴有头晕、恶心呕吐、出汗甚至虚脱。
3.有面部潮红、全身热感。
4.偶发皮疹，甚至剥脱性皮炎。
禁忌
1.脑出血或头颅外伤，因本品可使颅内压增高。
2.严重贫血患者应用本品，可能加重心脏负担。
3.青光眼患者禁用，因本品可增高眼内压。
4.心肌梗死患者有低血压及心动过速者。
5.肥厚梗阻性心肌病者应用硝酸盐可使其左室流出道压差增加。
6.严重肾功能损害。
7.严重肝功能损害，可增加变性血红蛋白危险。
8.在婴幼儿的胃肠道中，硝酸酯迅速转成为亚硝酸酯，后者可迅速吸收且对血红蛋白毒性非常大。
注意事项
1.连续使用硝酸异山梨酯时，部分患者的抗心绞痛效果和对血循环的作用会迅速产生耐药性。长期用药为保持疗效需增加剂量。
2.长期使用硝酸异山梨酯乳剂的患者，临时静脉注射该药的效果明显下降。
3.交叉过敏反应，对其他硝酸酯或亚硝酸酯过敏患者，亦可能对本品过敏。
4.长期使用本品血中硝酸盐类增多，变性血红蛋白也可能增加，尿中儿茶酚胺(肾上腺素和去甲肾上腺素)与尿中3-甲氧基4-羟基苦杏仁酸(VMA)值显著升高。
5.应用本品过程中应进行血压和心功能的监测。
6.用药期间从卧位或坐位突然站起时需谨慎，以避免突发体位性低血压。 宜自小剂量开始，逐渐增量。
